Before your patient or client admits to all level of Santé’s recovery continuum of care, you are encouraged to be part of the admissions process. When a client admits and releases are signed, the Referral Communications Coordinator (RCC) will be notified of admission. With releases remaining in place, Santé’s RCC will ensure you receive the types and frequency of updates you need. We believe that the information shared will provide a smoother transition when the individual returns to you for aftercare. There is so much we can achieve together in this continuum of care.

The severity and complexity of addiction treatment necessitates individualized plans unique to each client. Many of our clients are dual diagnosis and poly-addiction; other programs couldn’t have met their specific needs. At Santé Center for Healing in Argyle, Texas, we work with industry professionals to accommodate clients who will benefit more from our recovery continuum of care. The goal is long-term recovery.

Continuum of Care

Sante Center for Healing’s continuum of care slowly and intentionally evolved and developed over 25 years based on research and studies indicating participation in formal treatment and longer terms in treatment are consistently associated with better outcomes.

 

Countless clinical studies over the years, and most recently the “Addiction Treatment Aftercare Outcomes Study” published in the Open Journal of Psychiatry, determined that the longer an individual is engaged in addiction treatment, the better the success for sustained recovery. The American Society of Addiction Medicine asserts the chronic nature of the illness mandates a coordinated, science-based approach for chronic disease management.

 

Santé offers individuals and their families a long-term, clinical recovery continuum of care spanning more acute medical and clinical needs to less acute therapeutic transitions.

 

Santé’s Masters level Intake Counselors conduct an initial and confidential assessment to establish the appropriate level of care, whether it is Santé’s medical detox, residential program, intensive outpatient, transitional living, or intensive workshops… followed with life-long alumni support.

In all levels of care:

  • An individualized treatment plan is developed and evolves.
  • Progress milestones are gauged by individual goals achieved (not merely in days in care)
  • Services are received in an intimate, therapeutic, serene and supportive environment.
  • Families receive necessary education and empowerment for the life-affirming possibilities found in long-term recovery.

These are critical ingredients in the recovery process, increasing the likelihood that gains made especially in residential treatment are reinforced and sustained for life-long recovery.
